I see a lot of patients with shoulder pain. Rather than ordering an MRI study to further evaluate a questionable labral tear or supraspinatus impingement, I would rather order a MSK ultrasound. Yet...it seems that interpretation of these studies is not a U.S. radiologist skillset. European radiologists moreso, US radiologists, not so much. Thoughts? Disagree?

It is so good to be back in the arena, taking care of patients! I enjoyed my time in academic medicine and interacting with future colleagues, seeing them succeed and enjoying their professional lives.Now, at 8:11pm I am finishing up some notes and reflecting on how much I am enjoying time with my patients!Patient care is a gift that I will never take for granted.Wishing the best to my colleagues!

So many times I am asked by patients in their 40s and 50s to order an MRI for their routine low back pain. Atraumatic, nonradicular, weeks-old low back pain. Gonna have to take a hard pass on ordering this MRI.I say no lovingly, with a smile and an explanation, but I say no nonetheless.

My stellar students at National University of Health Sciences continue to contact me about specializing in Orthopedics...they are asking me about what exams they would have to pass after they complete the orthopedic program to achieve board certification. Things have changed a lot since I completed my ortho training...I had to take a 400-question written exam followed 6 months later by an oral practical exam. Good news...this is no longer necessary...check the website for the International Academy of Neuromusculoskeletal Medicine website for info.
